Adult Services - Post covid assessment serviceThis service supports people post-COVID-19 or post-viral infection. The service is based on rehabilitation and therapy to support self-management recovery. The service is mainly in people's own home, in groups or via remote support. The team includes clinicians and care coordinators with a range of experience in supporting rehabilitation in short and long-term conditions.
Patients are usually assessed by an occupational therapist, a physiotherapist or a nurse within 6 weeks of referral, however at times of high demand this may take longer. NHFT also work with system partners, such as, local GP practices and hospitals to make sure patients are supported.
Who is this service for? People who have had COVID-19 or a viral infection, whether diagnosed or not, where they experience ongoing health issues as a result of their illness.
How do I register for this service?
This service requires a GP referral.
Anyone concerned that they might have had COVID-19 or a viral infection that is not improving as expected, should seek advice and support from their GP surgery. Their GP team will be able to assess them and may offer a range of tests that are tailored to their individual signs and symptoms. If appropriate, they will make a referral to this service.

Meet the team
Psychological therapies clinicians and care coordinators can support you with a referral for psychological therapy. Interventions offered are evidence based and could include cognitive behavioural therapy, acceptance and commitment therapy, compassion based therapy, eye movement destabilisation and reprocessing or guided self-help.
Physiotherapists support patients with initial assessments, breathing techniques, general mobility and strengthening exercises to improve your day-to-day function.
Occupational therapist support patients with initial assessments and work with patients who may have difficulties carrying out day-to-day activities due to long COVID.
Care coordinators assist with the patient pathway in the community to make sure patients are well supported with their suggested plans for rehabilitation.
Community nurses support patients with initial assessments, look at the overall progress of their symptoms and consider alternative levels of support, which may include referrals to other services if signs and symptoms indicate this is needed. Assessments offered could include palpitation or dizziness assessment if Postural Orthostatic Tachycardia Syndrome or other forms of orthostatic intolerance is suspected.
