Community Mental Health TeamThe Community Mental Health Team (CMHT) in NHFT provide local assessment, monitoring, support, and treatment for a full range of mental health conditions. The CMHT is for people with mental health needs which cannot be met by NHS Talking Therapies, the GP and third sector agencies.

The CMHT is for people with mental health needs which can be met by members of the MDT which cannot be met by NHS Talking Therapies, the GP and third sector agencies.
How do I register for this service?
Accessing our service
Patients are usually referred into the Community Mental Health Team service from a GP or another Mental Health Service area.

Meet the team
The CMHT is made up of a Multi-Disciplinary Team (MDT). The MDT has a variety of different professionals in it including Mental Health Nurses, Medical staff, Psychologists, Assistant Psychologists, Occupational Therapists, Peer and Carer Peer Support Workers, Social Workers and Community Support Workers. Each different member of staff has different skills and come together to help each other and to ensure that the service user’s needs are met which will improve their outcomes.
The teams are supported by Clinical and Team administrators.
