HPV Vaccination Consent Form

Please complete the consent form whether you want your child to have the vaccine or not. If the form is not returned, your child may be given the opportunity on the day to self-consent. Please be assured that if we receive a written parental refusal of consent (indicated on this consent form), we will not vaccinate your child.

If your child is not in Year 8 and you think they may be due, or missing, any school age vaccinations, please contact the School Aged Immunisation Service admin hub on 0800 170 7055 (option 5). Lines are open 08:30-16:30, Monday to Friday.
